Catchment Area
The doctors welcome new patients who live within our practice catchment area. As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor.
You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms can be downloaded below, or available from reception:
Medical Treatment
Medical treatment is available from the date of registration but if you are on regular medication please ensure you have 2 weeks worth of medication from your previous GP Surgery. Please contact reception for further information.
Health Check
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
